How do I connect with a Family Care Team?

STEP 1: Individuals can register for Patient Connect NL online or by telephone.

STEP 2: Once registered on Patient Connect NL, you will be contacted by telephone when there is availability to connect you to the family care team in your area. You are now considered a patient of the Family Care Team.

Once you are connected to a family care team, you can call to discuss your health needs.
